Family Practice Medicine  Opportunity – Any State License accepted

Specialty:  Board Certified or Board Eligible Family Practice

Client:  Indian Health Service

Location: Kayenta, AZ

Dates coverage is needed:  asap – 6 months (with option to extend)

Schedule/Shifts Times:  Monday – Friday, 8am – 5pm

Call Requirements: none

Patient Volume:  12-15 patients per day average

Patient Ages:  all ages

Types of Cases: outpatient

Support Staff: yes – Specialty Physicians, mid-levels, nurses and admin support

Type of Charting: EMR – RPMS

Requirements:  BLS, ACLS, PALS (through American Heart), DEA (schedules 2-5)

Reason for Need: staff shortage

Credentialing Timeframe:  4-6 weeks on average
